Keto and Type 2 Diabetes: What Recent Studies Show

Disclaimer: this article is informational and does not replace medical advice. If you are diabetic and on insulin or glucose-lowering medications, talk to your doctor BEFORE changing your diet. Carb reduction may require immediate medication adjustments to prevent severe hypoglycemia.

The core mechanism: low insulin, stable blood sugar

Type 2 diabetes stems from insulin resistance. Your cells stop responding properly to insulin, so the pancreas pumps out more and more, until it exhausts itself. Keto attacks the problem at the root: fewer carbs in = less insulin secreted = sensitivity gradually restored.

What recent studies show

Virta Health Study (2018-2023)

262 type 2 diabetic adults followed under a medically supervised ketogenic protocol. 2-year results:

  • Average HbA1c dropped from 7.6% to 6.3% (remission threshold: < 6.5%)
  • 60% of participants stopped or reduced their diabetes medication
  • 94% reduced or stopped insulin
  • Average weight loss: 12% at 1 year, maintained at 2 years

Indiana University Meta-Analysis (2022)

Analysis of 14 randomized trials. Conclusion: very low-carb diet (< 50 g/day) is more effective than low-fat for reducing HbA1c, fasting glucose, and insulin needs in type 2 diabetics.

Absolute precautions

If you take insulin or sulfonylureas

Real risk of severe hypoglycemia from week 1. Your doctor must adjust doses alongside carb reduction. Self-monitor blood glucose 4-6 times per day during transition.

If you take metformin

No hypoglycemia risk, but monitoring is wise. Many patients see their medication reduced or stopped by their doctor after 3-6 months.

If you take SGLT2 inhibitors (Jardiance, Farxiga)

Increased risk of euglycemic ketoacidosis. Keto may be contraindicated. Medical consultation mandatory.

Pillar foods

  • Proteins: eggs, fatty fish (salmon, sardines, mackerel), poultry, lean red meat
  • Healthy fats: olive oil, avocado, nuts, butter, hard cheese
  • Vegetables < 5 g carbs/100 g: spinach, broccoli, cauliflower, zucchini, cucumber
  • Strictly avoid: sugar, bread, pasta, rice, potatoes, sweet fruit (except small berry portions)

Concrete 4-week action plan

Week 1: drop to < 50 g net carbs/day. Measure blood glucose morning and evening. Track how you feel.

Week 2: drop to < 30 g. Reinforce electrolytes to avoid keto flu. Book a doctor appointment for medication review.

Weeks 3-4: stabilize. Add 30 minutes of walking after each meal. Lab work recommended at 3 months.
